Patients with Original Medicare Part B, seen in the last 3 years Exclude Medicare Advantage and SCO plans.

Of those, patients with two or more chronic conditions Diabetes, hypertension, heart failure, COPD, CKD, depression and similar, on the problem list.

Of those, patients who also have Medicaid (QMB / dual eligible)

Patients currently billed for chronic care management (CCM) 0 if you don't bill CCM. They can switch, with new consent.
